Handy Forms for Counselors

These awesome counseling forms can be found and downloaded from a wonderful school counselor's webpage called Elementary School Counseling. I used these forms this past year to help me stay organized and to help me keep track of all my meetings, phone calls, things I needed to do and students I needed to see for individual counseling sessions, and I would highly recommend them for your school counseling binder.



 
 
Another great BlogSpot that has FREE resources and forms for counselors is The Not So Wimpy Teachers Manual.  My favorite one is the Communication Log form that I've attached below.
 
 

If you are looking for a handy form to help you keep up with individual students like the one I have pasted below, I would recommend The Helpful Counselors BlogSpot.
 
The next form is one that I created for when I meet with a student for the first time. It helps breaks the ice and gives me the opportunity to get to know the child a little better before we start talking about whatever issue they are having at home or at school. Under the "Summary" heading, there are five pictures. Each picture stands for a different topic. The house represents who lives in their home. This tells me a lot about the child's family dynamics and home life. The hands symbol represents the child's friends at school and at home. This lets me know who they are "hanging out with', what influences (positive or negative) they may be receiving from others, etc. The smiley face stands for the thing(s) they like about school and the sad face stands for their least favorite thing(s) about school. Finally, the person on the bike represents the things they like to do when they are not at school. For example; fishing, going to the beach, riding their bike, etc.
